What Is OPCAB Surgery?
OPCAB stands for Off-Pump Coronary Artery Bypass. It is a technique for performing heart bypass surgery without stopping the heart or using a heart-lung (cardiopulmonary bypass) machine.
In conventional bypass surgery, the heart is stopped and a machine temporarily takes over the circulation while the surgeon works. In OPCAB surgery, the surgeon operates on the beating heart using specialised stabilisation equipment that holds the target area still while the rest of the heart continues to beat normally.
What Are the Advantages of Off-Pump Bypass?
For suitable patients, OPCAB surgery can offer several important benefits compared to conventional on-pump bypass:
- Reduced risk of stroke — avoiding the bypass machine eliminates the risk of micro-emboli associated with the pump circuit
- Lower risk of kidney injury — the kidneys are not exposed to the inflammatory response triggered by cardiopulmonary bypass
- Less blood loss — and reduced need for blood transfusion
- Faster recovery — patients often leave hospital sooner and return to normal activity more quickly
- Better outcomes in high-risk patients — particularly those with poor heart function, diabetes, kidney disease, or significant calcification of the aorta
Is OPCAB Suitable for Everyone?
Not all patients are suitable candidates for off-pump surgery. The technique requires significant surgical experience, and the decision depends on the individual patient’s anatomy, the number and location of arteries to be bypassed, and overall clinical condition.
In experienced hands, OPCAB can be performed safely in the majority of patients who require bypass surgery — including many who were previously considered too high-risk for conventional surgery.
